29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  At this establishment, employees engaged in chemical refining activities, including, but not limited to, mixing chemical batches and loading chemicals in tanker trucks, and were exposed to hazardous chemicals and fall hazards.  The employer did not ensure a hazard assessment was conducted by means of a written certification.
