29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:    (a) On the back northwest side of the building:  On or about 6/20/2018, the pump jack scaffold was set up and used, while only securing the scaffold to the top of the structure and not at the bottom.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    (a) On the back, northwest side of the building:  On or about 6/19/2018, the employer did not adequately train each employee who performed work from a pump jack scaffold in that, the employees worked from the scaffold without an end rail and did not recognize the hazard, exposing the employees to an 18 foot fall hazard.
