29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about June 20, 2018 the employer failed to ensure that the Galiso automatic cylinder inverter/ valver and inspection machine (serial number civ0812-0083f) was guarded to prevent employees from the pinch point between the valve adapter and cylinder while the machine was turning. A single actuation switch required only one hand to have the machine turn while the other hand is free to reach the pinch point.
