29 CFR 1926.404(b)(1)(ii): Ground-fault circuit interrupters. 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which were not a part of the permanent wiring of the building or structure and which were in use by employees, did not have approved ground-fault circuit interrupters for personnel protection: a) Fairlane Village Mall, employees used a demolition hammer, battery chargers, fans, and other 120VAC equipment, which was powered through extension cords, and the circuits were not protected by ground-fault circuit interrupters, on or about, June 22, 2018. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section: a) Fairlane Village Mall, an employee used a handheld powered chipping tool, Bauer Demolition Hammer Kit, to remove mortar during ceramic tile floor removal, and scrape concrete floor/mortar after partition wall removal. The unit was not equipped with a commercially available shroud and dust collection system, or equipped with a continuous water delivery system, on or about, June 22, 2018. ABATEMENT CERTIFICATION REQUIRED
