29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired:  a) Jobsite, 300 Frankfort Road, Monaca, PA:  On or about June 28 2018, HHM Double Sided Shell Buggys remained in use after numerous deficiencies were noted that affected their safe operation and one buggy in particular that was involved in a serious incident.

29 CFR 1926.454(c): The employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, and the employer did not retrain such employee so that the requisite proficiency was regained:  Jobsite, 300 Frankfort Road, Monaca, PA:  On or about June 28, 2018, employees did not receive training to operate the HHM Double Sided Shell Buggy until a second incident occurred with another buggy and still continued to not operate it in accordance to the manufacturer's specifications and the company training they had received.
