29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye;  a) 2252 E College Avenue State Collage, Pennsylvania - On June 15, 2018, an employee was hospitalized with a broken tibia/ fibia when a fork lift operator struck the employee as he was backing up. The employer failed to report this work-related accident to OSHA within 24 hours.  (The employer reported this hospitalization to OSHA on July 5, 2018.)  No abatement certification or documentation needed.
