29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a)  Side of New Warehouse - Employees working on gravel stop and coping were exposed to a fall hazard of approximately forty five (45) feet when they did not attach a lanyard to the basket of the aerial lift, on or about July 10th, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Roof - An employee moving roofing supplies from one side of the roof to the other was exposed to a fall hazard of approximately forty (40) feet without the use of a conventional fall protection harness or system, on or about July 10th, 2018.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.  The Donald B Smith Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(10), which was contained in OSHA inspection number 960031, citation number 2 item number 1 and was affirmed as a final order on 04/08/2014, with respect to a workplace located at 9300 Olde Scotland Road, Shippensburg, PA 17257.
