29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power presses: (a) Press Room - The Shadow VI light curtain used the point of operation safeguarding device on the V&O Super OBI 25-ton part-revolution mechanical power press, serial number 25ST-266, did not protect employees who operated the press by preventing and/or stopping normal stroking of the press if the operators inadvertently placed their arms, hands, or fingers in the point of operation because gaps existed between the front of the press' bolster plate and the bottom of the light curtain large enough for an employee to inadvertently get arms, hands, and fingers into the point of operation undetected by the light curtain, which an employee did on July 3, 2018, resulting in a finger tip amputation. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
