29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.      a) Production Area: On or about July 17, 2018, and at times prior, the employer exposed employees to fire hazards in that, the employer did not conduct periodic inspections of their energy control procedures to ensure that the requirements of this standard were met where equipment and machines were serviced, such as, but not limited to, the Mixer, Line 1 Fill Vibrator, Cement Screw Conveyor, Dust Collector, Cement Auger, Big Ass Fan (curing chamber), Reclaim Conveyer, and Line 1 Empty Mold Belt Conveyer.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.    a) Production Area - Mixer Area: On or about July 17, 2018, and at times prior, the employer exposed employees to fire hazards in that, a relocatable power tap installed on a Husky Tool Cart and 2 extension cords were daisy chained and installed for use with an industrial fan (Portable Cooler Model # PC 4815 Serial # C16), Computer system (Hope Industrial Systems Model HIS-UM19-CGBH), and LED Area Light (Model 9969).    The Environmental Materials, LLC DBA Environmental StoneWorks) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2) which was contained in OSHA inspection number 991425, citation number 2, item number 2 and was affirmed as a final order on January 26, 2015, with respect to a workplace located at 1341 Discovery Industrial Ct., Mableton, GA 30126.
