On or about July 17, 2018 in the pipe fab shop, employees were exposed to a struck-by hazard while using a below the hook lifting device that was not load tested when installed. 3. On or about July 17, 2018 in the Metallurgy Lab, employees were exposed to a struck-by hazard while operating two monorail mounted Coffing hoists that were not load tested when installed. 4. On or about July 17, 2018 in the Metallurgy Lab, employees were exposed to a struck-by hazard while operating a monorail mounted Budgit hoist that was not load tested when installed.
