29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  (a) New townhome construction on Lot #103, Front side: On or about July 19, 2018, and at times prior, the employer exposed employees to fall hazards of approximately 25 feet from the lower level, in that the scaffold work platforms were not entirely secured to the forks of the JCB All-Terrain Material Handler while the employee was installing OSB plywood sheathing onto a residential townhome structure.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) New townhome construction located on Lot #103, Front side: On or about July 19th, 2018, and at times prior, the employer exposed employees to ejection/fall hazards of approximately 25 feet from the lower level, in that personal fall arrest systems were not provided to employees installing OSB plywood sheathing on a work platform attached to a JCB All-Terrain Material Handler.
