On or about July 26, 2018, at a job site located at Lot 7853, Paradisio Dr., Galena, Ohio 43021, the employer failed to protect employees from head injuries while performing framing work on a two story residential home by not requiring the use of protective helmets.     The Trail Builders, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1096426 citation number 1, item number 1 and was affirmed as a final order on November 12, 2015 with respect to a workplace located at 2122 Township Rd 415, Dundee, OH 44624.

Trail Builders was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1096426, citation number 1, item number 3, and was affirmed as a final order on November 12, 2015, with respect to a workplace located at 4649 King Meadow, Kent, Ohio 44242.
