29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or warning line system and safety monitoring system:  Two employees were observed working on a low slope roof (4/12 pitch) without fall protection exposing themselves to fall hazards ranging from 8-15 feet to ground level.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  A Skillsaw brand 7-1/4" portable electric circular saw was plugged into an extension cord which at the plug end had the primary insulation pulled away from the strain relief devise exposing the secondary insulated conductors.
