This was most recently documented at 1351 S. Reca Ct, Wichita, Kansas, where employees were using forklifts to aid in the removal and replacement of the right and left Boom Hoist Cylinders on a Lull Highlander (Model 1044C-54). The employees were exposed in that the loads on the forklifts were not safely arranged and stable. a) On or about 07/27/18, a Nissan forklift (CPH02A20V) was used to facilitate the replacement of the right Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The slings and pins were added to cradle the load. No additional means were provided to prevent the load from becoming unstable or shifting throughout the process. b) On or about 08/03/18, a Toyota forklift (42-6FGU25) was used to facilitate the replacement of the left Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The slings and pins were added to cradle the load. No additional means were provided to prevent the load from becoming unstable or shifting throughout the process.

The employees were exposed in that additions and modifications that affected the capacity and safe operation of the forklift were added without the manufacturer's written approval. a) On or about 07/27/18, a Nissan forklift (CPH02A20V) was used to facilitate the replacement of the right Boom Hoist Cylinder on the Lull. In order to accomplish this task 8 foot slings were added to the forks using Countyline hitch pins (part#18PCH001TSC). The addition and placement of the slings and pins affected the safe operation of the forklift as the calculated center of gravity and lifting capacity of the forklift was modified.
