29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Banquet Prep Kitchen - On or about July 28, 2018, the employer did not ensure that the LaMonferrina pasta extruding machine was adequately guarded to prevent the operator from having any part of their body in the danger zone during operation. An employee suffered a finger amputation when the cover was opened during operation.   No abatement certification or documentation is required for this item.
