29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  a.  On or about August 17, 2018, employees engaged in residential construction activities at the jobsite located at 1119 E Roeland Ave Appleton, WI 54915 did not keep the work area free of scrap lumber with protruding nails, and all other debris.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a.  On or about August 17, 2018, two employees engaged in residential construction activities on the second story of a steep pitch roof, with unprotected sides and edges greater than fifteen feet above the ground at the jobsite located at 1119 E Roeland Ave Appleton, WI 54915 were not protected from falling by an acceptable form of fall protection.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.  a.  On or about August 17, 2018, employees engaged in residential construction activities on the second story of a steep pitch roof at the jobsite located at 1119 E Roeland Ave Appleton, WI 54915 did not use the eight foot folding ladder for the purpose for which it was designed.  Two employees used the step ladder in its folded position to access the second story.
