29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):    a). At the worksite - On or about August 15, 2018, employees worked from the building's roof while installing roof shingles without the use of a fall protection system, thus exposing the employee to a fall hazard up to approximately 10 feet.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:    a). At the worksite - On or about August 15, 2018, a Werner portable extension ladder did not have its side rails extended at least three (3) feet above the roof's landing surface, thus exposing an employee to a fall hazard of approximately 10 feet.
