29 CFR 1926.651(b)(2): The employer proceed with an excavation without the utility companies or owners establishing the location of the utilities underground installations prior to the start of actual excavation, and proceeded without caution, and provided detection equipment or other acceptable means to locate utility installations are used.    On or about August 23, 2018, the employer proceeded with an excavation without prior location of the utilities underground installations.  This condition exposed employees to fire and explosion hazards.
