An employee sustained a work-related injury on or about 3/12/18.  The employee was provided medical treatment, physician directed restrictions and was transferred to a less physically demanding client.  The employer did not record the injury on the 2018 OSHA 300 Log.    (b)  HomeCare of the Rockies Inc, at 7105 La Vista Place #100, Longmont, CO: On or about 6/5/18 the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  An employee sustained a work-related injury on or about 6/5/18.  The employee was provided physician directed restrictions.  The employer did not record the injury on the 2018 OSHA 300 Log.
