29 CFR 1904.7(b)(1): Employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional:    (a)  HomeCare of the Rockies Inc, at 7105 La Vista Place #100, Longmont, CO: On or about 3/12/18 the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  An employee sustained a work-related injury on or about 3/12/18.  The employee was provided medical treatment, physician directed restrictions and was transferred to a less physically demanding client.  The employer did not record the injury on the 2018 OSHA 300 Log.    (b)  HomeCare of the Rockies Inc, at 7105 La Vista Place #100, Longmont, CO: On or about 6/5/18 the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  An employee sustained a work-related injury on or about 6/5/18.  The employee was provided physician directed restrictions.  The employer did not record the injury on the 2018 OSHA 300 Log.    (c)  HomeCare of the Rockies Inc, at 7105 La Vista Place #100, Longmont, CO: On or about 7/16/18 the employer did not ensure that a work-related injury or illness was recorded if it resulted in one or more of the following: death; days away from work; restricted work or transfer to another job; medical treatment beyond first aid; loss of consciousness; or significant injury or illness diagnosed by a physician or other licensed health care professional.  An employee sustained a work-related injury on or about 7/16/18.  The employee was provided medical treatment and physician directed restrictions.  The employer did not record the injury on the 2018 OSHA 300 Log.
