29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a)  The employer did not ensure that an employee exposed to a struck-by hazard from above was protected by a protective helmet. The employee was exposed to the possibility of falling work tools and materials from above by an employee conducting roof edge installation.

29 CFR 1926.503(c):     "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where:
