29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:   a) On or about June 18, 2018, sanitation employees were required to work with All American Chemical Company's, Whistle Klean, a corrosive chemical (pH >11.2 undiluted).  The employer did not require and ensure the use of long chemical resistant gloves to protect affected employee's hands. The lack of proper gloves exposed the employee's hands to chemical burns.

Chicago Meat Authority, Chicago, Illinois - On or about August 22, 2018, the employer did not enter correctly the following workplace injuries or illnesses on the OSHA Form 300 or its equivalent for the calendar year 2018.     a) On or about June 15, 2018, a recordable injury occurred when a sanitation employee received a chemical burn injury to his right wrist while working in the south plant, resulting in 12 lost workdays. Not all portions of the OSHA Form 300 were completed, including the information requested in Column H (check if there were days away from work), Column K (enter the number of days away from work), Column L (enter the number of days of restricted duty), Column M 1 (check for injury).     b) On or about June 19, 2018, a recordable injury occurred when a sanitation employee received a chemical burn injury to his right hand while working in the warehouse, resulting in 40 lost workdays. Not all portions of the OSHA Form 300 were completed, including the information requested in Column H (check if there were days away from work), Column K (enter the number of days away from work), Column L (enter the number of days of restricted duty), Column M 1 (check for injury).
