The propane tank mount did not have an index pin to secure the tank.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)(: JOB SITE: 3901 Union Boulevard, Saint Louis, MO 63116, on or about 9/5/2018, workers were allowed to operate the Clark Forklift, MODEL: CGC 25, S/N: C365L-0257-9488FB with defective brakes. A safety check of the lift revealed the brakes did not engage on initial press of the brake pedal. The workers were exposed to a struck-by hazard.
