29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Curtain Department- On or about August 16, 2018, the employer did not ensure that the guard was properly adjusted to cover all unused portions of the SuLee Model: FA-200 automatic cutter blade or designed to protect employees from inadvertent contact with the danger zone during the operating cycle.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHAs public web site at www.osha.gov: a) Curtain Department- On or about August 16, 2018, the employer did not report to OSHA, within twenty-four (24) hours, a work-related amputation injury that occurred with the SuLee Model: FA-200 automatic cutter. The injury was reported on August 22, 2018.
