29 CFR 1926.651(c)(2): The employer did not ensure that a safe means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress was located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.    This violation occurred on September 6, 2018, when employees were exposed to cave-in hazards with no safe means of egress.  No ladder or ramp was located within 25 feet from where the employees worked at the base of a trench that was more than 7 feet in depth.

Protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:    This violation was observed on September 6, 2018, when at least two (2) employees were exposed to struck-by hazards while working at the base of a trench more than 7 feet in depth and where the excavated soil was not kept at least 2-feet back from the edge.
