A stairway, ladder, ramp or other safe means of egress was located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. This violation occurred on September 6, 2018, when employees were exposed to cave-in hazards with no safe means of egress. No ladder or ramp was located within 25 feet from where the employees worked at the base of a trench that was more than 7 feet in depth.
