29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck, the employer did not ensure that each operator had successfully completed the training required by paragraph (l):    On or about August 16, 2018 the employer did not ensure that an operator was certified by evaluation and trained prior to operating powered industrial trucks. This condition exposed employees to injuries resulting from, but not limited to, struck-by and caught-in-between hazards.

CFR 1910.178(l)(3)(ii):  The employer did not ensure that each operator of a forklift completed the training consisting workplace-related topics listed in 1910.178(l)(3)(ii)[A] - [I] of this section:      b) On or about August 16, 2018 the employer did not provide instruction and/or implement training ensuring that trailers at ramp-docking areas were chocked or adequately restrained at all times during loading or unloading operations.     This condition exposed employees to injuries resulting from, but not limited to, struck-by and caught-in-between hazards.
