29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent.  Traffic Masters Inc. Located at Douglas Road and CR 14 Fort Collins, CO 80525:  On and preceding 13 September, 2018, the employer did not use an OSHA 300, 301 and 300A Form or equivalent. The employer indicated that the company had up to 15 employees including part-time.  When the OSHA 300 logs were requested, the owner indicated that she was not familiar with OSHA logs or other injury and illness forms.
