29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. Jobsite: The employer did not ensure that an engineering survey of the structure was performed by a competent person to determine the conditions of the framing floors, walls and possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. On September 17, 2018, a deck on which an employee was working collapsed during the demolition process, and the employee fell to the lower level with the deck. The employee sustained injuries form the incident.

29 CFR 1926.855(b): Planks not less than 2 inches by 10 inches in cross section, full size undressed, were not provided for, and/or were not used by employees to stand on while breaking down floor arches between beams. Jobsite: The employer did not provide and mandate the use of planking for the employees to stand on while manually demolishing floor arches between beams. On September 17, 2018, a deck on which an employee was working collapsed during the demolition process, and the employee fell to the lower level with deck. The employee sustained injuries from the incident.
