29 CFR 1910.23(c)(4): Unsecured ladder(s) were used on surfaces that were not stable and level: a) Facility - On or about 09/17/18 and at times prior thereto employees performing maintenance at apartment #118 placed a Louisville 16 ft. extensible aluminum ladder on top of the 4:12 sloped porch roof, and climbed the unsecured ladder in order to access the main roof. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extended at least 3 feet (0.9 m) above the upper landing surface: a) Facility - On or about 09/17/18 and at times prior thereto employees working at Apartment #118 accessed the porch roof at 8.0 ft. above ground level with a Louisville 8.0 ft. fiberglass A-type (step) ladder, and the ladder rails did not pass three feet above the landing surface. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
