Bead Cutter, Serial # L15130 to cycle with only one of the two controls having to be pushed.     b) On September 25, 2018, the Renco Inc., Two Handed No-tie-down Control, Model REN23-3W installed on the CTD Machines, Inc. Bead Cutter, Serial # L15130 was not recessed far enough back to prevent employees from engaging both controls while portions of their hands were still in the point of operation.
