29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about September 26, 2018, the employer failed to perform an annual audit of their written Energy Control, Lock Out Tag Out program to ensure that it was updated and still effective.  The last time this program was reviewed was in 2015 and new equipment has been added to the facility within this past year.

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures:  On or about September 26, 2018, the employer failed to ensure that authorized employees have been retrained in the Energy Control, Lock Out Tag Out program after new equipment/machines were installed at the facility within this past year.  The last documented training of authorized in employees in this program was in 2015.
