29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about October 1, 2018, employees engaged in demolition activities were exposed to:  a) slipping hazards while working from a wet transite roof and the employer did not instruct employees in the recognition and avoidance of unsafe conditions while working on transite; and  b) struck-by hazards while using pry bars and hammers to remove nails from the transite roof and the employer did not instruct employees in the recognition and avoidance of unsafe conditions while working on transite.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about October 1, 2018, employees performing demolition activities on a roof were exposed to a fall hazard of approximately 20.6 feet and the employer did not ensure a suitable fall protection system was installed for each employee.
