On or about October 10, 2018, Beer and Liquor Warehouse: Access to the side-hinged emergency exit at the back of the warehouse was blocked by the placement of Michelob Ultra 12-packs of beer, a grocery cart, a trash can, a 12.5 gallon Hoover wet/dry vac, and a push broom head.

On or about October 10, 2018, Beer Warehouse:  Employees were allowed to operate a Husky Prime Mover battery-powered pallet stacker without having received formal instruction, including operator evaluation to determine the retention of the information.
