29 CFR 1926.652(a)(1): Employees were not protected in an excavation from a cave-in by an adequate protective system designed in accordance with paragraph (b) and (c) of this section.  (a) At the construction site located near Brown Deer Rd and 76th St, Milwaukee WI, employees were performing work in an approximately 7 feet 2 inch deep excavation without adequate cave-in protection, thereby exposing employees to a struck-by / crushing hazard.
