29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       a.) Charter Plastics, Inc., Charter East-Machine #12 area of the facility, on or about October 19th, 2018 and times prior thereto:  Employees were exposed to struck-by hazards while working with/around the sidewinder pipe coiling machine (Sakas-NMC brand #CQL78-S1084).  The sidewinder coiling machine was not guarded to protect employees from potential struck-by or caught-in hazards.       b.) Charter Plastics, Inc., Charter East-radial saw/storage area,  on or about October 25th, 2018 and times prior thereto:  Employees utilized a Sears Craftsman 10" radial saw (Manufacturer # I-80-7) that was not equipped with a guard to cover the exposed portion of the saw blade.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not effectively enclosed:       a.) Charter Plastics, Inc., Winding machine #7 area, on or about October 19th, 2018 and times prior thereto:  The sprocket and chain associated with winding machine #7 were not effectively enclosed in that, there was an approximate 3.5 inch opening on the guard exposing employees to a caught in hazard.

Charter Plastics, Inc., on or about October 19th, 2018 and times prior thereto:  An amputation was not reported to OSHA within 24 hours.  The incident, and resulting amputation, took place on or about September 5th, 2018.  The incident was not reported to OSHA until October 12th, 2018.       b)  Charter Plastics, Inc., on or about October 19th, 2018 and times prior thereto:  An amputation was not reported to OSHA within 24 hours.  The incident, and resulting amputation, took place on or about September 20th, 2018.  The incident was not reported to OSHA until October 12th, 2018.
