William Miller dba Millers Construction, 1450 West 12th Street, Erie PA  16501, on or about November 8, 2018 and times prior thereto:  Employees were exposed to fall hazards off of and through a personnel platform in the elevated position.  Employees were utilizing a JLG G5 18A rough terrain forklift truck and a wooden pallet to lift personnel to the roof of the structure.  Employees were exposed to a fall hazard of 12.5-feet to the ground below.
