29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a)  Trenchless Solutions, Inc.; Alleyway behind 510 East Beckwith Avenue, Missoula, MT:  On or about November 7, 2018 and at times prior, employees were exposed to struck by hazards while installing a new septic line with the spoil pile placed less than 1 foot from the edge of the open trench.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:   a)  Trenchless Solutions, Inc.; Alleyway behind 510 East Beckwith Avenue, Missoula, MT:  On or about November 7, 2018 and at times prior, employees were exposed to struck by hazards while installing a new septic line with the competent person not conducting a daily trench inspection to include items such as, but not limited to: evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, and other hazardous conditions.  Employees were allowed to enter the trench to install the new sewer line without the hazards being corrected.

29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:  a)  Trenchless Solutions, Inc.; Alleyway behind 510 East Beckwith Avenue, Missoula, MT:  On or about November 7, 2018 and at times prior, employees were exposed to crushed by hazards while installing a new septic line with all voids around the outer edge of the trench box not being filled to prevent lateral movement.
