29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:   a)  Trenchless Solutions, Inc.; Alleyway behind 510 East Beckwith Avenue, Missoula, MT:  On or about November 7, 2018 and at times prior, employees were exposed to struck by hazards while installing a new septic line with the competent person not conducting a daily trench inspection to include items such as, but not limited to: evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, and other hazardous conditions.  Employees were allowed to enter the trench to install the new sewer line without the hazards being corrected.
