29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) The employer did not ensure that employees were using fall protection when conducting work from an elevated platform during framing carpentry work on a residential building.

29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section. (a) The scaffold work platform used to elevated employees on the New Holland Telehandler rough terrain forklift was not designed by a qualified person.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) The employer did not ensure that an elevated platform used to support persons, material and equipment was attached to the forks of the New Holland Telehandler rough-terrain forklift (model no. LM840CTPTMR, serial no. 041717653).
