This exposed employees to a struck by hazard.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) Forklift #2023 (Yale): On or about November 27, 2018, a front light was burnt out. (b) Forklift #2025 (Yale): On or about November 27, 2018, a rear backup light did not work. (c) Forklift #2026 (Yale): On or about November 27, 2018, a red light bar was not working and the propane tank was not mounted on the positioning bolt. (d) Forklift #2029 (Yale): On or about November 27, 2018, a front light was burnt out. (e) Forklift #2031 (Nissan): On or about November 27, 2018, rear brake lights did not operate, front turn signal light did not operate, and the propane tank positioning bolt was missing. (f) Forklift #723 (Toyota): On or about November 27, 2018, a caution strobe light was not working.
