On or about November 16, 2018, at the jobsite located 401 Bussen Underground Road, Saint Louis, Missouri, an employee was hospitalized for an injury sustained while operating Toyota Electric Forklift Truck (Model No. 8BN CU18; Serial No. 55200). The employer failed to report the hospitalization within 24 hours as the report was received on November 21, 2018.
