29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) Paulding County Adult Detention & Law Enforcement Center, Storm line C located in the southeast corner: On or about 12/05/2018, and at times prior, the employer did not instruct the employees who were working in a 8-ft. deep x 16-ft. long trench box to recognize potential hazards related to cave-in hazards which included the 22-ft. unprotected soil wall and the sloped walls at the top of the trench box which were not placed at least 18-inches below the top of the box.

29 CFR 1910.651(k)(2): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1910.651.    (a) Paulding County Adult Detention & Law Enforcement Center, Storm Line C, Southeast Corner: On or about December 5, 2018, and at times prior, employees were exposed to engulfment hazards in that employees were not removed from the excavation where hazardous conditions were present by the competent person, while employees were working inside a trench shield.
