Site: 2251 Plumb 1st Street Brooklyn, NY    On or about 12/3/18    a) An employee used a 20 foot Louisville extension ladder to access an outside light fixture and the ladder was not secured to prevent movement.  An employee was injured when he fell from the ladder.
