29 CFR 1910.178(n)(6): The forklift operator did not keep a clear view of the path of travel:    A)    Cheeseman, LLC, located at 2200 State Route 119, Fort Recovery, OH 45846: On or about December 6, 2018, the employer did not ensure that the operator of the Crown C-5 forklift # 68 (Serial # 10041286) kept a clear view of the path of travel when the operator was backing out of the trailer which was parked at the loading dock door # 59. Employees were exposed to struck-by and crushing hazards.
