On December 12 2018, at a jobsite located at 3303 South Oxford Ave, Independence, MO 64052, an employee conducting work at the bottom of a trench was exposed to serious electrical injuries. The extension cord energizing the power tools was not utilizing a ground fault circuit interrupter.

On December 12, 2018 at a jobsite located at 3303 South Oxford Avenue, Independence, Missouri 64052, an employee was exposed to serious struck by and or engulfment hazards. There was no ladder or other safe means of egress to and from the open excavation approximately 6 feet 10 inches in depth.

On December 12, 2018, at a jobsite located at 3303 South Oxford Avenue, Independence, Missouri 64052, an employee was exposed to serious struck by and or engulfment hazards. There was standing water which continued to accumulate in the open excavation due to not having an effective means of water removal at the work site.

On December 12, 2018 at a jobsite located at 3303 South Oxford Avenue, Independence, Missouri 64052, an employee working in a 6 foot 10 inch excavation was exposed to serious struck by and or engulfment hazards. The spoil pile had not been placed at least two feet from the edge of the excavation. The spoil pile surrounded the trench and was right on the edge of the excavation.
