Compressed gas cylinder that were not in use, were not secured to prevent damage in accordance as required by Compressed Gas Association Pamphlet P-1-1965 in the following location: a) On December 21, 2018, three compressed gas cylinders located next to the automatic welder were not secured. No abatement certification or documentation is required for this item.

There were no controls in place to monitor the concentration of lead in the return air.
