Employees were exposed to serious amputation, crushing and struck-by injuries, in that employees had not been trained to understand and demonstrate the knowledge and skills required to safely lock and tag out the energy sources, including but not limited to the 1900 Mold machine pan pusher.

Employees in the mold area were exposed to a amputation hazard or other serious injuries to the fingers and hands, in that the Sprocket Wheels and Chain on the 1900 Mold machine hydraulic pump located less than 7 feet from the floor were not enclosed.
