29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):    (a)  437 700 ROAD, NEW OXFORD, PA, NORTH SIDE - Employees performed soffit replacement work from a ladder jack scaffold, and the ladder access to the West side of the scaffold plank where the employees accessed the scaffold was immediately adjacent to the unguarded electrical service connected to the house, on or about January 3, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    (a)  437 700 ROAD, NEW OXFORD, PA, NORTH SIDE - Employees performed soffit replacement work from a ladder jack scaffold, and the West side of the scaffold plank extended over the West support approximately five feet, on or about January 3, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    (a)  437 700 ROAD, NEW OXFORD, PA, NORTH SIDE - Employees performed soffit replacement work from a ladder jack scaffold at a height of approximately 20 feet, and the employees who erected the scaffold were not knowledgeable of basic scaffold erection and use requirements and strategies, on or about January 3, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
