29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On or about January 15, 2019 and times prior thereto, employees the employer did not an effective hearing conservation program for employees exposed to an 8-hour time weighted average(TWA) of more than 85 decibels, exposing employees to noise hazards in the following instances:  a) Baggers were exposed to a 8 hour TWA of 88.1 dbA  or 77.6% dose. The time sampled was 405 minutes with zero exposure assumed for the 75 minutes not sampled.  b) Floaters were exposed to a 8 hour TWA of 88.4 dbA. or 80.6% dose. The time sampled was 436 minutes with zero expsosure assumed for the 44 minutes not sampled.  c) Maintenance/ shredder operators were exposed to an 8 hour TWA of 87.3 dbA or 69.7% dose. The time sampled was 424 minutes with zero exposure assumed for the 56 minutes not sampled.  The following elements shall be included: (d) Monitoring (e) Employee Notification (f) Observation of Monitoring (g) Audiometric Testing (i) Hearing Protectors (k) Training (m) Recordkeeping

29 CFR 1910.303(e)(1)(ii): Electric equipment was used without markings giving voltage, current, wattage, or other ratings as necessary:  On or about January 15, 2019 and times prior thereto, the circuit breakers in the back of the main shop that supplied power to the equipment were not labeled, which exposed employee to the electric hazards of shock.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  On or about January 15, 2019 and times prior thereto, the electrical cabinets were blocked by a blue cart containing propane and handtools, which exposed employees to electrical shock hazards in the event of an emergency.
