29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared:  A) An employee, front department CNC machine operator, was exposed to continuous noise levels at 70.0% of the allowable eight (8)-hour time weighted sound level of 90 decibels (dBA).  The equivalent dBA level of 70.0% is approximately 92.1 dBA.  The sampling was performed for 165 minutes on February 6, 2019. (Zero exposure was assumed for the unsampled period of time.) A baseline audiogram was not established within six months of the employee's employment, on or about, February 6, 2019.     ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels:    A) An employee, hydraulic forging press operator, was exposed to continuous noise levels at 68.3% of the allowable eight (8)-hour time weighted sound level of 90 decibels (dBA).  The equivalent dBA level of 68.3% is approximately 87.6 dBA.  The sampling was performed for 165 minutes on February 6, 2019. (Zero exposure was assumed for the unsampled period of time.) The employee's annual audiogram had not been obtained within the last calendar year, on or about, February 6, 2019.    B) An employee, roto blast operator, was exposed to continuous noise levels at 70.4% of the allowable eight (8)-hour time weighted sound level of 90 decibels (dBA).  The equivalent dBA level of 70.4% is approximately 92.7 dBA.  The sampling was performed for 163 minutes on February 6, 2019. (Zero exposure was assumed for the unsampled period of time.) The employee's annual audiogram had not been obtained within the last calendar year, on or about, February 6, 2019.    C) An employee, roto blast operator, was exposed to continuous noise levels at 72.5% of the allowable eight (8)-hour time weighted sound level of 90 decibels (dBA).  The equivalent dBA level of 72.5% is approximately 93.3 dBA.  The sampling was performed for 165 minutes on February 6, 2019. (Zero exposure was assumed for the unsampled period of time.) The employee's annual audiogram had not been obtained within the last calendar year, on or about, February 6, 2019.  ABATEMENT CERTIFICATION REQUIRED
