29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) Building Exterior: An employee was observed operating an aerial lift and wearing a personal fall arrest harness, but  not secured to the aerial lift basket exposed to fall hazards of approximately 30ft to the concrete below. Condition noted on or about 01/28/2019  b) Building Exterior: An employee was observed operating an aerial lift and wearing a rappelling harness and not a personal fall arrest harness exposed to fall hazards of approximately 30ft to the concrete below. Condition noted on or about 02/04/2019
