1926.959(d)(3)(iii)(D): Each employee was not protected from hazards that could arise from mechanical equipment contact with energized lines or equipment. Employing insulating protective equipment or barricades to guard against any remaining hazardous electrical potential differences. a) Worksite: On or about February 7, 2019, and at times prior, the employer exposed employees to electrical shock and burn hazards in that proper personal protective equipment was not provided while working near energized power lines.

29 CFR 1926.1408(a)(2)(iii)(B): Determine if any part of the equipment, load line or load (including rigging and lifting accessories), while operating up to the equipment's maximum working radius in the work zone, could get closer than the minimum approach distance of the power line permitted under Table A (see �� 1926.1408). If so, then the employer must follow the requirements in paragraph (b) of this section to ensure that no part of the equipment, load line, or load (including rigging and lifting accessories), gets closer to the line than the minimum approach distance. (a) Worksite: On or about February 7, 2019, the employer exposed employees to electrical shock and burn hazards in that an Altec Telescopic Boom Truck Crane (Model AC45-127S Serial Number 0114FS0151) did not maintain a 10 foot clearance from the boom load line to energized 7,200 volts overhead power lines.
